No, thats wrong. The knox infection will appear as "infected" on a wound, the same way a regular infection does. You cant tell the two apart on the menu itself, but you CAN'T be infected without a bite, scratch, or laceration from a zombie. Unless you get a infected tag on an injury that you can't clear just by disinfecting and using sterile bandages you shouldn't need to use antibiotics until you get a sickness noodle at the second stage or so. Just like injuries, infections severity is random withing a set range, so save those antibiotics until you are certain you need them.
